About The Awards

Notice

To submit an entry for the 2008 ActSafe Awards you will need to provide us with the following information:

  • The name and details of the person submitting the entry
  • The name and details of a second workplace contact person
  • Your company's name and details
  • The Award Category your entry is for
  • The title of your solution or initiative
  • A general description of your company's business
  • Confirmation that your business has not been prosecuted for an OHS offence or experienced a work-related fatality in the past few years
  • An overview of your solution or initiative and comments in respect of each of the judging criteria for the Category you are entering
  • Any additional electronic attachments (each entry MUST be accompanied by at least one relevant photo; other relevant material may also be submitted in support of your entry)

Make sure you have all of this information ready before you proceed to create your entry.

Once you are satisfied that your entry is complete, a copy will be emailed to us and a second copy will be emailed to you for your records.

NOTE:

You will not have the ability to save your application once you have started. Should your entry be particularly wordy, or complex, or should you wish to compose it over several sessions, you should consider completing your application in a word processor, using this form as a guide, then copying and pasting into the online form.
